If two people answer the same control question differently during interviews, that inconsistency becomes a finding.


Gap #2: Tools Are Deployed but Not Operationally Enforced

Many organizations invest heavily in security tools to meet technical requirements. This is necessary, but it is not sufficient.

Assessors routinely find:

  • Logging tools collecting data that no one reviews

  • Monitoring alerts configured but ignored

  • MFA technically enabled but bypassed through exceptions

  • Endpoint protection installed but not centrally monitored

From an assessment perspective, a control that is not actively used, reviewed, or enforced is treated as partially implemented at best.

CMMC emphasizes operational effectiveness. Tools must demonstrate ongoing use, oversight, and response.


Gap #3: Evidence Exists but Is Not Assessable

Evidence collection often becomes a scramble late in the process, even when the organization believes it has everything it needs.

Common evidence issues include:

  • Screenshots without timestamps or context

  • Logs that cannot be tied back to a specific control

  • Evidence stored across multiple systems with no clear mapping

  • Evidence that exists, but no one knows where it is during interviews

Assessors are not looking for volume. They are looking for traceability.

Evidence must clearly show:

  • Which control it supports

  • How it demonstrates implementation

  • That it reflects ongoing practice, not a one-time snapshot

When evidence cannot be quickly explained or mapped, assessments slow down and risk increases.


Gap #4: Control Ownership Is Unclear or Assumed

Assessors frequently ask a simple question that exposes a major weakness:

“Who owns this control?”

In many organizations, the answer is unclear, shared, or assumed to be “IT.”

Common problems include:

  • Controls owned informally, not formally assigned

  • Ownership split across departments without coordination

  • Backup owners not identified

  • Owners unaware they are responsible for the control

Without clear ownership:

  • Controls degrade over time

  • Evidence collection becomes fragmented

  • Interview responses vary

  • Accountability is lost

Mature organizations explicitly assign ownership for every control and ensure owners understand their responsibilities.


Gap #5: Staff Awareness Does Not Match Documentation

CMMC assessments include interviews because behavior matters.

Assessors often discover that:

  • Staff are unaware of certain policies

  • Training occurred once but was never reinforced

  • Employees follow informal processes instead of documented ones

  • Teams rely on “tribal knowledge” rather than defined procedures

This gap is especially risky because it cannot be fixed quickly. If staff behavior does not align with documentation, assessors will question whether controls are truly institutionalized.

Consistency across people, not just systems, is a core expectation.


Gap #6: Processes Work in Theory but Not Under Real Conditions

Some controls look solid until they are tested under pressure.

  • Backup and recovery processes that were never validated

  • Access reviews that are scheduled but routinely delayed
